Referrals to Other Services

We may refer you to other service providers. Referrals will not be made without prior discussion without your written consent.

Whānau Ora Services and Your Data

  • Whānau Ora begins with a discussion about whānau aspirations and it is your choice to engage. Whānau who decide to participate will co-design their own plans. The plans are not decided by the Government, Commissioning Agencies, or providers. If a whānau chooses not to create a plan or set goals, they cannot access Whānau Ora supports.
  • Your plan is a key source of information that helps us understand the overall impact of Whānau Ora. Your individual and whānau data are confidential to your service provider and Commissioning Agency. The Government, including Te Puni Kōkiri and Stats NZ, does not have access to your personal information or the contents of your plan.
